    In Connecticut and most states fees are set by statute. In Connecticut where I practice attorneys charge 20% of the recovery.

    If you were not at fault it may be improper to raise your rates. Whether the insurance company can deny med pay benefits depends on the language in your insurance policy.

    Physical therapists can be held liable for medical malpractice just as doctors. Generally physical therapists must follow physicians' orders. The failure to do so may indicate malpractice.

    To establish medical or dental malpractice you need to obtain your medical and dental records and have them reviewed by an expert, most likely a dentist. If that expert concludes that your injury resulted from a deviation from accepted standards of care then you may have a malpractice lawsuit.

    In Connecticut small claims cases are generally not appealable. Sounds like you had your day in Court. If there is some claim not addressed in the small claims judgment you may be able to pursue it in a civil lawsuit.
