Report your injury to your employer in writing immediately (as soon as you can). You have 30 days, but the sooner the better. Print out a Form 18, fill it out and sign it and make two copies. Keep one copy for yourself and send the other to the NC Industrial Commission.

What to do if the employer fails or refuses to report an injury.
Call 919-807-2506 (Industrial Commission) and get a Form 18 (which you can get from the Industrial Commission). They will assign you a file number.
Mail the form 18 to: NC Industrial Commission, Statistics Section 4334 Mail Service Center, Raleigh, NC 27699-4334.
Mail the first copy to your employer certified mail, return receipt requested, and keep the receipt (as proof). Keep a copy for yourself at home.

Medical Treatment
The employer usually sends injured workers to their official medical provider. If the employer refuses to help you with medical treatment, you can choose your own physician/hospital, but must promptly request the Industrial Commission's approval. For example, you are severely injured and then go to the ER, and then go to your family doctor - you must let the Industrial Commission know otherwise you can jeopardize your WC claim.
