Designated Doctors resolve impairment and other medical related disputes in the comp system. They are required to be qualfied to address the medical issues they are assigned to examine or review. Their qualifications are presented on a "matrix." This guide explains how to obtain that matrix.

Format
The request must be in writing and be addressed to the Office of the Medical Advisor. It may be mailed to Texas Department of Insurance, Division of Workers' Compensation, 7551 Metro Center Drive, MS-41, Austin, TX 78744-1609. It may be faxed to (512) 804-1040. It may be emailed to wcopenrecords@tdi.state.tx.us

Specify That You Are Making An Open Records Request
The request should clearly state that you are making an open records request or a public information request.

Specify What You Want
The request should next clearly explain what document you are requesting. It should indicate the name of the doctor, the doctor's license number (if you have it), and indicate that you want a copy of that doctor's matrix that was submitted to show his qualifications for Division-directed exams, including those for impairment ratings.

Response From DWC
You should receive a response from the Division in approximately ten to twelve business days.
