Specify That You Are Making An Open Records RequestThe request should clearly state that you are making an open records request or a public information request. 3
Specify What You WantThe request should next clearly explain what document you are requesting. It should indicate the name of the doctor, the doctor's license number (if you have it), and indicate that you want a copy of that doctor's matrix that was submitted to show his qualifications for Division-directed exams, including those for impairment ratings. 4
