THE PEOPLE OF THE STATE OF NEW YORK TO: SUFFOLK COUNTY POLICE DEPARTMENT COMMANDER 6TH PRECINCT WE COMMAND YOU, that all business and excuses being laid aside, you and each of your appear and attend before District Court Suffolk County located at 400 Cartelton Avenue, Central Islip, New York, 9:00 AM in the fore noon, and at any recessed or adjourned date to give testimony in this action on the part of the Plaintiff in this matter and that you bring with you, and produce at the time and place aforesaid, all and any certain documents now in your custody, and all other deed, evidences and writings, which you have in your custody or power, concerning , including but not limited to complaint reports, follow up arrest reports, reports, investigations, and or complaints and/or any and all other records in relation to said Failure to comply with this subpoena is punishable by contempt of Court and shall make you liable to the person on whose behalf this subpoena was issued for a penalty not to exceed fifty dollars and all damages sustained by reason of your failure to comply.
