Contact Your State Regulatory Agency and File a Complaint
Nursing homes are regulated very heavily. In most states, there is a state agency charged with investigating complaints of nursing home abuse and/or neglect. In Illinois for example, the Illinois Department of Public Health investigates complaints and has a toll free number for filing complaints.

Consider Contacting an Attorney Who is Familiar with Nursing Home Abuse Cases
An attorney who is familar with nursing home cases will most likely have additional contact information and other resources within your state. For example, Illinois has a group of Ombudsman who are specifically charged with being liasons between consumers and nursing homes. An attorney may also be able to give you further advice on the processes in your state.

Request the Nursing Home Chart
The sooner you are able to obtain your loved one's nursing home records, the better. HIPAA laws are very strict on who may obtain medical records. Should your loved one pass away prior to obtaining his or her records, this becomes even more difficult.
