And what should they say? Observe your reactions to soundbites of news: who died and how, who has been brought back to life? Focus on your own desired outcomes, so that your own wishes can be met, as much as possible. 2
Who is the SMARTEST person you know? Who likes to THINK?Who likes to receive new information? And uses it to solve problems? Who likes to make decisions? Who can re-make a decision fairly easily? Who do you know who enjoys THINKING? 3
Who is the KINDEST person you know? Who has kind FEELINGS?Who is kind to others? And you? Can this person follow directions? Tactfully? Will your desired communications get lost if this person is To Tactful? 4
Who is the BEST COMMUNICATOR you know?Who is able to "say what s/he means and mean what s/he says"? Is this person able to speak from YOUR script? Whose wishes will be communicated? Sometimes it is hard for a spouse to serve, because of their own impending Loss. 5

Now you are EITHER ready to prioritize these 3 individuals as your Health Care Agent, OR, you may want to select a Professional Fiduciary:You can complete the FIVE WISHES form to state your wishes. In doing so, you will gather thoughts showing what Your Particular Agent's 'job description' looks like. When you name people to serve as your Agent, share copies of the form with each of them, so that each one has a copy and each one knows your wishes.
