USCIS requested a visa for me nearly three weeks ago and I haven't heard anything yet

Asked about 1 year ago - West Palm Beach, FL

The officer that interviewed me requested a visa for me and I haven't been approved yet, should I be concerned? My priority date is current and has been current since I filed.
