    Once your divorce is final you cannot be insured by your wife’s employer. She has to pay for your insurance. Talk to your lawyer about this.

    While this probably falls outside the standard of care, you did not state that it is bothering you in any way. Without any current damages no attorney would take on this case since you have no real damages, and damages are what drives malpractice cases.

    Most of the major insurers require your SSN nowadays because your rates are determined, in part, by your credit score(s).

    The dentist’s office can bill whatever they want. If the dentist is in your insurance network, then they are contractually bound to accept the $17.00 paid for the fluoride charge and not bill you for the difference. As long as you are not being charged for the write-off amount then you should be OK. However, if the office is trying to bill you for the write-off amount then this is called “balance billing”, and it violates the insurance agreement between your insurance company and the dentist’s office.

    If balance billing is occurring then you have to notify your insurance company ASAP.

    It is in their financial interest to have insurance, but there is no requirement forcing them to have insurance other than to insure the building if the property is mortgaged.
