    What you really need to do is hire defense counsel immediately. The insurance company is obligated to provide counsel at their expense if you get sued, although nothing prevents you from seeking counsel now. As far as the affidavit, you have no obligation to provide information.

    You have both a statutory and contractual duty to cooperate. If you fail to cooperate, your insurance company may deny your benefits, but you probably already know this as they will send you notice of intent to deny. Hire counsel!

    Sounds like they were negligent in not keeping a safe place for their customers, however, damages might be an issue. You say that you sustained an injury to your shoulder so you should see a doctor immediately, and then contact a personal injury attorney.

    I am sorry but I have to agree with the other lawyers here that you cannot have passed the field sobriety test, otherwise the officer would not have asked you to take a breathalyzer test, additionally, the field sobriety, like the Star Trek Spock test, is meant to be failed. The amount is way over the limit and it could be conducive to enhanced penalties, so you should consult a competent DUI attorney immediately. Your license will be suspended but there are ways to get it back but time...

    Congratulations, you won your trial (or someone you know did). Acquitted means to find someone innocent, to release or discharge of criminal charges. Evidence received in vault means that whatever evidence was used at trial was returned to the evidence room for safekeeping.

    I wouldn't be surprised if the insurance company claims accord and satisfaction, however it would be very hard for them to prove it. Not sure why you were doing this without an attorney, and I am a little worried about what you may have sent in writing to the insurance company. Always get an attorney when dealing with an insurance company, they surely have theirs by their side.
