What happens if the judgment is never recorded at the Recorder office ? Can it still be treated as a lien ?

No; no lien against real property until/unless abstract of judgment is recorded.

FYI... also, it may be possible for a judgment lien to be avoided in a bankruptcy case, in addition to the underlying debt/lawsuit/judgment being discharged.
