I have talked to a lawyer but unfortunately since the respondent is not paying me any child support & the money I bring in isn't even enough to pay the bills, I don't have money for the retainer.

there's probably a form but more importantly than the form is to know what you're entitled to and to know what to ask for. if you don't do the petition correctly, make the proper pleadings and requests for relief and so forth, you will be precluded from getting the relief needed. Forms are not a real good substitute for knowing what you're doing.
