I was simply inquiring whether there are specific forms or terms that must be included to comply with state law and to properly effectuate what we have already resolved in our minds?

A short answer - if you have agreed to terms there are no forms or terms I know of to be filed with the state and your CPA can advise you how to handle the tax treatment and filing.
